STUDENT PHYSICAL & SHOT RECORDS
Each child must have a physical examination every year given by a physician or nurse practitioner recorded on Florida State Student form. These forms are provided by the health care provider and must be completed before the child is accepted and then annually. Each child must also have an updated immunization record prior to admission, which must be kept current while the child is in the child care setting.

Parents are responsible for informing staff and administration of known allergies and/or special conditions of a child.



ILLNESS
   
In order to help ensure healthy conditions for your child, as well as others, we ask you to please keep your child at home and telephone the child care office if any of the following conditions exists:

• Fever of 100 degrees or more, must remain out next day.

• Running, Red Eyes or pink eye (conjunctivitis) must remain out next day.

• Ear Ache, if running 

• Green Discharge from nose

• Skin Rash or Sores

• Enlarged Glands

• Diarrhea, stay must remain out next day.

• Unusual Fatigue and/or Irritability

• Vomiting, must remain out next day.

• Severe Cough or Sore Throat

Also, please keep your child at home if he/she has contracted communicable/contagious conditions such as:

• Chicken Pox Measles Lice Mumps

• Strep Throat Impetigo

• Scarlet Fever Ring Worm Flu or Severe Cold

Parents are asked to assume the responsibility of keeping a child at home if there is any doubt about the child's health. Whenever your child has been exposed to a communicable disease, please report it to the child care office. Children sent home with any of the above symptoms are asked to stay out the following day.

When your child becomes ill or injured at school, no treatment other than first aid will be administered. If the illness or injury is serious, you will be notified immediately. An incident and/or accident report will be written by the staff member who saw the situation, signed by the Director, and a copy made available to the parent.

All children that are ill or injured (where applicable) will be isolated, while keeping him/her under observation, until the parent can take the child home or to the doctor. Parents are expected to pick up their child immediately.

MEDICAL EMERGENCY 
In any situation where a child requires immediate care we will call 911 and the parents.

There may be times when medical attention may be necessary for your child. Every attempt will be made to contact you first, which is why it is vital for you to notify us of any changes in your schedule (work or school telephone numbers or locations, emergency information, etc.)

In the event of an injury warranting a physician's attention, the Center will assist you in getting your child and yourself to the appropriate facility.

INCIDENT/ACCIDENT REPORT    
Any incidents of injury to a child, illness or unusual behavior are documented on an incident report and signed by staff, parents and administration on the day of incident. All necessary follow up is documented on the report.  This report is then placed in student’s permanent file.
